In accordance with the Home Office Statutory Guidance for the granting and renewal of certificates, applicants may be required, at their own expense, to obtain a GP report as part of the application process. The medical form is included at the end of the F116 form and available to download.
A medical declaration is not required if the person making the application holds a valid firearm and/or shotgun certificate with at least three years validity remaining and their medical circumstances have not changed during the validity of the certificate.
The application will only be processed upon receipt of the application form (and GP report). Please allow plenty of time for you to obtain your GP report. Where possible please submit the application form and GP response together to minimise delay in the application process.
